The authors report on 404 Southwest Asian refugees seen at a community clinic, Approximately three-quartes of these patients met DSM-III criteria for major depressive episode, and 14% had posttraumatic stress disorder. Complaints of pains and sleep disturbances were the predominant presenting symptoms. Most of the men were married, but more than 40% of the women were widowed. Between 15% and 30% of the patients reported specific traumatic eperiences either in their homeland or during their escape. Widowhood and such traumatic experiences were positively correlated with more symptoms of depression and anxiety,

The unique nature of combat in Vietnam, in particular the opportunities provided for behaviors by soldiers that could induce guilt (such as the killing of civilians), made veterans of that war particulary likely to develop Post-Traumatic Stress Disorder (PTSD). Our studies have found suicidal behavior and preoccupation with suicide to be disproportionately frequent among Vietnam combat veterans with PTSD and to be related not so much to having engaged in nonmilitary combat behavior as to feeling out of control due to fear or rage while doing so. The combat histories of veterans who did not develop PTSD show a common cluster of affective and behevioral traits that appear to have been protective. The military policies that shaped the way war was fought in Vietnam, the reception many soldiers faced on their return home, and clinician's frequent failure to recognize and effectively treat PTSD, all contributed to the long-term dicfficulties experienced by so many Vietnam veterans.

For the asylum seeker the process is typically fraught with frustration, connfusion, and difficulty in dunderstanding what is legally required by the host country, in determining their status , and pehaps future well-being.

This chapter reviews the evidence for the existence of a complex form of postraumatic disorder in survivors of prolonged, repeated trauma. This syndrome is currently under consideration for inclusion in the diagnostic and Statistical Manual of Mental Disorders - Fourth (DSM-IV) under the name of Disorders of Extreme Stress Not Otherwise Specified (DESNOS). The current diagnostic formulation of Post-Traumatic Stress Disorder (PTSD) derives primarly from observations of survivors of relatively circumscribed traumatic events. This formulation fails to capture the protean sequelae of prolonged, repeated trauma. In contrast to a single traumatic event, prolonged, repeated trauma of the perpetrator. The psychological impact of subordination to coercive control has many common features, wheter it occurs whthin the public sphere of politics or within the private sphere of sexual and domestic relations.
