We present a review of the international literature on the medical and psychological effects ot torture. Our review reveals that certain tortures and their physical and emotional sequelae are more prevalent than previously appreciated. They include the common occurrence of sexual violence during the torture of women and female adolescents and the high frequency of head injury and associated neuropsychiatric consequences. We recommend the use of standarxized diagnostic criteria in the evaluation of patients who have survived tortureé; this will facilitate patient care and the documentation of human rights violations.

Our understanding of the psychosocial and cultural dimensions of disease and illness is limited not merely by a lack of empirecal knowledge but also by an inadequate medical semantics. The empiricist theorie of medical language commonly employed both by comparative ethnosemantic sudies and by medical theory are unable to account for the symbolic contexts. A semantic network analysis conceives the meaning of illness categories to be constituted not primarily as an ostensive relationship between signs and natural disease entities but as a 'syndrom' of symbols and experiences which typically 'run together' for the members of a society. Such analysis directs our attention to the patterns of association which provide meaning to elements of amedical discourse to articulate distinctive configurations of social stress and to negociate relief for the sufferer. This paper provide a critical discussion of medical semantics and develops a semantic network analysis of 'heart distress, a folk illness in Iran.

Les différences de priorisation de ces problèmes sont marquées, alors que les familles priorisent les problèmes de santé des enfants, les intervenants priorisent ceux des mères et plus particulièrement leurs problèmes de santé mentale. Familles et intervenants expriment ensuite des souhaits de consultations du réseau formel de services; il s'agit, pour les familles, de consultations de médecins et de pédiatres alors que ^pour les intervenants, il s'agit de médecins et d'infirmières. Ces personnes sont ensuite effectivement consultées. Lorsqu'elles ne le sont pas, les raisons de non accessibilité sont essentiellement d'ordre linguistiques. Les difficultés de compatibilité des services sont finalement perçues comme plus considérables par les intervenants que par les familles.

Accumulating evidence from studies investigating the role naturally-occurring emotional support networks play in remediating psychological distress strongly suggests that such networks buffer personal distress and lessen the need for formal mental health care. Research findings also suggest that reliance on emotional support networks varies across ethnic groups. The present study compared emotional support network characteristics of Anglo-Americans to those of Mexican americans, in addition to examining the relationship between the reliance on specific support providers and psychopathological symptoms for Anglo- and Mexican Americans. Randomly selected adult Anglo- and Mexican Americans (n=515) living in one of three suburban communities in Southern California provided responses to a standardized measure of psychological impairment and named those persons on whom they depended in time of personal problems. Both Anglo and English speaking Mexican American (ESMA) respondents reported significantly larger networkers and more cumulative contact and reciprocity with network members than did Spanish-speaking Mexican Americans (SS MA) respondents. Anglos and ES MAs named significantly more friends and neighbors as emotional support providers than id SSMAs. SS MAs, on the other hand, more often depended on extended kin and spouses than did the other two subgroups. Professional caregivers accounted for a very small proportion of reponses across the subgroups. The number of friends and neighbors as support providers was significantly and positively correlated with problems of social relations in only the SS MA subgroup. Implications of results for community mental health direct and indirect service components are discussed.

The Cordelia Foundation for the Rehabilitation of Torture Victims has been active in Hungary since 1996. The foundation, within the frame of a ''mobile centre'', deals with the rehabilitation of traumatized war victims in several refugee camps. The staff -a psychiatrist, a non-verbal therapist, a human rights activist, and interpreters- enter the space of client in the psychological and physical meaning as well.


The therapies used at the Cordelia Foundation are adapted to the ''special situation'' of a refugee camp. We use verbal and non-verbal methods both individually and in groups. The patients originate from Europe, mostly former Yugoslavia, and from several non-European countries (about 30).

The members of the working group elaborated a special non-verbal therapeutic method called ''animation therapy'' (to animate unanimated objects). The therapist has used this method at psychiatric departments for several years, and adapted it to the rehabilitation of trauma victims.

The RCT Manual for the Rehabilitation of Torture and Organised Violence Survivors is addressed to health workers, particularly those who have no professional training and work in countries with limited resources. It constitutes a valuable tool and guide in the therapy not only for victims of Torture and Organised Violence, but also those of other psychosocial traumas as well. It represents more than two decades of the RCT's professional experiences, centred on torture survivors and refugees, and is also the result of active and open exchange via workshops and the RCT's joint, interactive work in Lantin America, Africa and Asia.

This manual has been developed by the RCT professional and technical team under the direction of Professor Bengt Sjölund for use by personnel who are not professionally trained in therapy, especially in countries with limited resources. It has an interdisciplinary and integrated approach and contains physical, physiological, pharmacological, psychological, social, psychiatric and therapeutic aspects, not only from an academic outlook, but also taking into account complementary or alternative medecine and respect for cultural, religious and spiritual aspects.
