"Educational Tools for Dealing with Trainees with Difficulties"

Iain Lamb

Associate Adviser in GP

SE Deanery.

This booklet contains a range of ideas tools and models that I have found useful in dealing with the many types of difficulty that can occur for learners, teachers, trainers and supervisors.

It should be used with the relevant Deanery and NES booklets giving practical and operational advice about managing difficulties during training.

SOME THOUGHTS ON THE SKILLS TO MANAGE LEARNERS IN DIFFICULTY

GOOD COMMUNICATION SKILLS

The skills we use to consult well are also those that help when communicating with registrars. You will all be familiar with various consultation models detailing a range of useful skills. Please refer to the “Communication Skills Booklet” for more details.

However I’d like to flag up 3 to give you thoughts about YOUR behaviour, to give a way of thinking about problems and ways we intervene.

MODIFICATION OF HELMAN FOLK MODEL (1)

  • What has happened?

  • Why has it happened?

  • Why has it happened to this trainee?

  • Why has it happened now?

  • What would happen if you did nothing about it?

  • What should I do about it and who should I consult for advice and help?

SIX CATEGORIES OF INTERVENTION (2)

  • Prescriptive Give advice or instruction—directive

  • Informative Explaining and giving information

  • Confronting Giving feedback to the registrar on their

behaviour or attitude, in order to help them

see what is happening

  • Cathartic Helping the registrar release their emotions

  • Catalytic Encouraging the registrar to explore their own

feelings and reason for behaviour

  • Supportive Encouraging the registrar’s self worth by giving

giving approval

TRANSACTIONAL ANALYSIS (3)

When dealing with difficulties do you do this in an Adult-Adult way or Parent-child??

TRANSACTIONAL ANALYSIS

Transactional Analysis (or TA as it is often called) is a model of people and relationships that was developed during the 1960s by Dr. Eric Berne. It is based on two notions: first that we have three parts or 'ego-states' to our 'personality. The other assumption is that these converse with one another in 'transactions' (hence the name). TA is a very common model used in therapy and there is a great deal written about it.
